Job summary

Jefferson Health is seeking a professional nurse committed to patient- and family-centered care. The RN will develop plans of care with patients and designated care partners, addressing individual needs and respecting cultural beliefs and values throughout the care journey.

Essential responsibilities include applying evidence-based practice, educating patients and families, and reducing risk by following standards and using health IT.

Qualifications

• Specialized Diploma from an accredited school of nursing.
• Bachelor of Science in Nursing preferred within 3 years of hire.
• Experience as an RN in an acute care setting preferred.
• Knowledge of patient- and family-centered care.
• Familiarity with health information documentation.

Responsibilities

• Integrates Evidence-Based Practice with clinical expertise and patient preferences for delivery of optimal care.
• Partners with patients and families to identify learning needs and provide education.
• Minimizes risk by following nursing practices and protocols and using information technology to mitigate error.
• Uses data to monitor care outcomes and design improvements for quality and safety.
• Functions within nursing teams, maintaining open communication and shared decision-making for quality care.
