Job summary

Responsibilities

• Assess patient needs and plan individualized care.
• Develop and revise the plan of care with input from patient and family.
• Provide direct patient care and supervise activities of the health care team.
• Evaluate patient progress and adjust care as needed.
• Document assessments and interventions accurately in the patient record.
