Job summary

Stairways Behavioral Health in Erie, PA is seeking a Rehabilitation Coordinator for the RTFA program on the 3rd shift. You will coordinate and implement treatment plans, assist with referrals, conduct intakes, gather information to develop strategies, and organize program discharges.

Bachelor’s degree with at least two years of related full-time (or equivalent) experience or Master’s degree with at least one year of related full-time (or equivalent) experience.
Valid driver's license with access to a vehicle during scheduled work hours.

Responsibilities

Coordinate and implement client treatment plans for RTFA residents.
Assist in coordination of referrals and conduct client intakes.
Gather information needed to develop treatment strategies and organize discharges.
Crisis management and resolution are key functions of this role.
Maintain professional relationships with internal staff and community resource providers; participate in weekly case review/treatment team meetings.
