Job summary

Hackensack Meridian Health is seeking an experienced Pharmacist for an overnight shift at our healthcare system in New Jersey. You will dispense medications, monitor therapy, and provide clinical services to ensure safe medication use.

Qualifications

• Graduate of an ACPE accredited school of pharmacy, or equivalent, with a minimum of a BS degree in Pharmacy.
• Education, Knowledge, Skills and Abilities Preferred: 1-2 years hospital pharmacy experience.
• Knowledge of USP 795, 797, and 800 standards; EPIC experience.

Responsibilities

• Complies with and enforces all state and federal regulations and policies relating to pharmaceutical care and medication management.
• Review and verification of all medications prior to dispensing from the pharmacy department; check allergies, polypharmacy, interactions, and clinical appropriateness.
• Oversee dispensing, chain of custody, and accountability of controlled substances.
• Handle investigational drugs at applicable sites.
• Adhere to USP 795, 797, and 800 standards for sterile and nonsterile compounding and hazardous drugs.
• Oversee ancillary personnel (Pharmacy Technicians and Assistant/Couriers).
• Provide safe medication practices as per guidelines and policies.
• Identify patient needs and adapt care to age, culture, language, etc.; communicate with patient and caregivers.
• Provide pharmacological information and answer requests from healthcare professionals.
• Participate in clinical initiatives (renal dose, IV to PO, PK dosing, antibiotic stewardship, opioid stewardship).
• Train pharmacy staff and residents; educate students and interns during rotations.
• Complete competencies, licensing, and certifications; conduct unit inspections for med storage and security.
• Perform other duties as assigned; adhere to organizational standards.
