Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Knowledge of patient- and family-centered practice and ability to engage diverse patients in care planning.
• Cultural awareness and ability to connect with patients from diverse backgrounds.
• Effective communication concepts, tools and techniques; communicate with patients and families.
• Knowledge of patient assessment and care; ability to implement a treatment plan.
• Knowledge of discharge planning and arranging follow-up care.
• Knowledge of health information documentation in written or electronic form.

Responsibilities

• Interacts with patients, families and staff to deliver optimal care using evidence-based practice.
• Identifies learning needs with patients and families and provides education.
• Uses information technology to communicate, document, and monitor care outcomes.
• Works within nursing and inter-professional teams with clear communication and mutual respect.
• Supports ongoing professional growth and precepting of staff members.
• Participates in hospital and setting committees focused on safety and quality.
