Job summary

Jefferson Health is seeking a professional nurse to provide patient- and family-centered care using the nursing process. Develops nursing plans of care with the patient and designated care partner(s) to address individualized needs and priorities, aligned with cultural beliefs and values.

The nurse will maintain high standards of safety and quality, collaborate with inter-professional teams, and participate in education, discharge planning, and continuous professional development.

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Knowledge of how to convey respect and collaborate with each patient, family and/or designated care partner in care and care planning.
• Knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively.
• Understanding of effective communication concepts, tools and techniques, with partnership language for patients and families.
• Knowledge of patient assessment and care and ability to implement a treatment plan.
• Knowledge of discharge planning and arranging for follow-up care.

Responsibilities

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff with Jefferson Health values in mind.
• Partners with patients and families in identifying learning needs and provides education.
• Uses information technology to communicate, manage knowledge, and monitor care outcomes.
• Works within nursing and inter-professional teams to deliver safe, compassionate care.
• Supports learning and acts as a preceptor where appropriate.
• Participates in safety and quality improvement committees and incorporates feedback.
