Job summary

Responsibilities include administering medications, monitoring residents, documenting care, and collaborating with the interdisciplinary team to ensure quality resident outcomes. California LVN license required; overnight availability preferred.

Qualifications

• Active California LVN license is required.
• Experience in skilled nursing or long-term care is preferred.
• Strong clinical, organizational, and communication skills.
• Ability to work independently during overnight hours.

Responsibilities

• Provide direct nursing care to residents per physician orders and care plans.
• Administer medications and treatments accurately and safely.
• Monitor residents' conditions and report changes promptly.
• Maintain accurate and timely nursing documentation.
• Collaborate with the interdisciplinary care team to ensure quality resident outcomes.
• Uphold all facility policies, procedures, and regulatory requirements.
