Job summary

Working with the Health and Wellness Director, you will lead patient-focused care, supervise staff, and help maintain compliant care plans, shifting from 11p-7a as a night shift. This role offers growth opportunities and a supportive environment focused on resident dignity.

Qualifications

• A valid LPN/LVN license is required.
• Minimum 4 years geriatric nursing experience.
• At least 1 year of supervisory experience preferred.
• Experience with memory care is a plus.
• Must meet all mandatory health requirements according to state regulations and participate in on-call rotation.

Responsibilities

• Perform resident assessments and reassessments according to policies, monitoring changes in condition.
• Communicate with Health and Wellness Director about resident needs to ensure quality of services.
• Provide hands-on care, medication administration, and specific nursing tasks as needed.
• Schedule and assign caregiving and med tech shifts to ensure coverage and compliance with staffing guidelines.
• Supervise, train, and support care team members and promote teamwork.
• Assist in transcription of physicians' orders and review new orders.
• Maintain accurate resident records, including care plans and incident reports.
• Coordinate care planning with home health agencies onsite within policies.
• Address residents' needs, requests, and complaints.
• Coordinate admissions with the sales team and supervise move-ins.
• Understand community care regulations and state-specific requirements.
• Facilitate communication between residents, families, and healthcare providers.
