Responsibilities

• Follow the nursing process: assess, plan, implement, and evaluate.
• Assess and analyze data including physical, psychological, sociocultural, spiritual, economic and lifestyle factors.
• Plan measurable short and long term goals with the care team.
• Implement and modify the plan of care with coordination, education and discharge planning.
• Document all phases of the nursing process in the patient’s record.
• Collaborate with an interdisciplinary team to optimize patient outcomes.
