Job summary

Porter is expanding into New Jersey and is looking for Registered Nurses to join our care team in in-home health risk assessments and medical management. You will conduct history reviews, vital signs, medication reconciliation, and comprehensive exams to support care plans for Porter Care Guides.

Qualified candidates have a New Jersey NP license, at least 3 years of patient care experience (preferably in primary care/adult/geriatric) and EMR skills.

Qualifications

• Valid unencumbered New Jersey NP license.
• 3 years of patient care experience required (primary care/adult/geriatric, EMR skills preferred).
• Strong empathy, compassion, and communication with patients and coworkers.

Responsibilities

• Visit members within your geographic area and conduct an in-home health risk assessment.
• Evaluate past medical history, review systems, and perform a comprehensive physical exam.
• Perform vital signs, medication reconciliation, and identify diagnoses for care management.

What You'll Do

You will visit members within your geographic area of responsibility and conduct an in-home health risk assessment including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam. This evidence-based practice screening will include point-of-care testing (as appropriate), the identification of diagnoses to be used in care management by our Porter Care Guides and active medical management in the furtherance of treatment, and the formulation of a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.

What You'll Need

• 3 years of patient care experience required (experience in primary care/adult/geriatric, and EMR skills preferred)
