As a Home Health Nurse (RN/LPN), You Will:

• Provide in-home direct patient care according to the prescribed plan of care, monitoring progress.
• Assess and document patient conditions, report changes (vitals, blood/stool/urine, glucose, etc.).
• Administer medication, insulin, IV fluids, and document thoroughly.
• Inspect and care for wounds, change dressings, assist with personal hygiene.
• Assess for injuries, infections, bedsores, and deliver appropriate treatments.
• Collaborate with healthcare team to ensure optimal patient outcomes.

Qualifications:

• Graduate of an accredited nursing school.
• Current unrestricted RN or LPN license in the state.
• CPR/AED/BLS/First Aid certification.
• Reliable transportation.
• Trach/ventilator experience preferred but not required.
