Qualifications

Responsibilities

• Conduct ongoing assessments of patients' physical and emotional needs.
• Develop nursing care plans including problem identification and discharge planning.
• Work as part of an interdisciplinary team to achieve rehabilitation goals.
• Ensure documentation in medical records is complete and accurate.
