  • Creates and implements prioritized care plans with projected outcomes.
  • Maintains continuity of care and timely documentation in charts.
  • Triages calls, educates patients and families on medications/treatment.
  • Performs in emergency situations following established standards.
  • Participates in education, quality improvement, and policy adherence.
