Interim HealthCare Inc. in Cincinnati, OH is seeking a Licensed Practical Nurse (LPN) to provide in-home patient care with 1:1 attention and dignity. You will follow the prescribed plan of care, monitor progress, and collaborate with other health professionals to deliver quality outcomes.

The role requires an active LPN/LVN license in Ohio, CPR/BLS certification, and reliable transportation to care sites. Flexible scheduling includes full-time and part-time opportunities, with daily pay options

Qualifications

• Graduate of a practical (vocational) nursing program for LPN/LVN and holds a current LPN/LVN license in the relevant state.
• CPR/AED/BLS/First Aid certification is required.
• Reliable transportation to/from care sites and/or work locations.

Responsibilities

• Provide in-home direct patient care according to the prescribed plan of care and nursing scope of practice.
• Assess and document patient conditions and report changes as needed (vital signs, etc.).
• Administer medication, insulin, and IV fluids with thorough documentation.
• Inspect and care for wounds, change dressings, assist with hygiene.
• Collaborate with providers and health professionals to ensure patient outcomes.

As a Licensed Practical Nurse (LPN) you will:

• Providein-homedirect patient care according theprovider’sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery.
• Assessand document patient conditions and reportchangesas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV fluids:documenting thoroughly.
• Inspect and care for wounds, changing dressings and assisting with personal hygiene.
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.

To qualify for a Licensed Practical Nurse (LPN) position with us:

• Education: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state(s) in which candidate completed at least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trach and/or ventilator experience preferred, not required
