Corewell Health in Grand Rapids, MI is seeking a Nurse Navigator for stroke patients to provide coordinated inpatient and outpatient care, education, and follow-up across settings.
You will perform mRS follow-up calls for intervention patients, coordinate carotid board care, and lead quality improvement projects while supporting patients and families.
Nurse navigator for stroke patients. Includes inpatient and outpatient work. Current responsibilities include modified Rankin Scale (mRS) follow-up calls for all intervention patients, carotid board care coordination, early targeted stroke education, and post-hospital follow-up coordination.

There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.
Responsible for providing quality education, follow-up and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.
