Responsibilities

• Performs PT assessment, reassessment and treatment; supervises PT assistants for pediatric to geriatric patients referred by a physician.
• Develops plan of care including goals and treatment plan in accordance with physician referral and injury/illness status.
• Modifies plan of care based on patient response and status changes.
• Maintains written documentation for all PT services, including initial assessment, follow-ups, and discharge summaries.
• Co-signs physical therapist assistant documentation biweekly.
• Provides verbal and/or written home instructions to patient and/or family.
• Communicates patient status to referring physician and health care team.
• Schedules patients to maintain productivity and ensure delivery of quality care.
