Infinite Medical P.C. is seeking a Neurology Nurse Practitioner/Physician Assistant to provide comprehensive neurology care in skilled nursing facilities across our network.

You will manage treatment plans, medications, and patient education for residents with epilepsy, MS, Parkinson’s, and other neurologic conditions. You will collaborate with a multidisciplinary team, perform thorough assessments, and maintain meticulous documentation while participating in quality improvement initiatives to

Qualifications

• Minimum 6 months of experience in long-term care or skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA License.
• CPR BCLS/BLS certified.
• Must hold a Master's Degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• A year of experience in Neurology.
• Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided.
• Must have great organizational and time management skills.
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.

Responsibilities

• Develop and implement individualized treatment plans for patients, including medication management and non-pharmacologic interventions.
• Conduct regular follow-up visits to monitor patient progress, adjust treatment plans as necessary, and provide education and support.
• Collaborate with physicians and other healthcare professionals to coordinate care and ensure a multidisciplinary approach.
• Provide patient and family education on disease management, medications, and lifestyle modifications.
• Maintain accurate and timely documentation of patient encounters and treatment plans.
• Participate in quality improvement initiatives to enhance patient outcomes and optimize patient care delivery.
• Perform comprehensive assessments to accurately diagnose and treat patients with neurological conditions.
