Job summary

Community Care Physicians is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Neurology in Niskayuna, NY. This established neurology practice offers collaboration with experienced neurologists and APPs while developing expertise in outpatient neurology.

The role involves evaluating and managing patients with diverse neurological conditions, performing comprehensive histories and exams, and coordinating care with the broader team.

Qualifications

New York State license and active registration as a PA or NP.
Current CPR certification.
PA: Bachelor's or equivalent degree and NYSED-approved program; NCCPA certification; ambulatory experience.
NP: Graduation from an NP program; certification by AANPCB or ANCC; pharmacology coursework; ambulatory nursing experience; CPR.

Responsibilities

Evaluate, diagnose, and manage neurological patients.
Perform comprehensive histories and neuro examinations.
Develop and implement treatment plans.
Manage acute and chronic neurological conditions.
Order and review labs, imaging, and tests.
Educate patients and manage medications.
Coordinate care with PCPs and specialists.
Maintain EMR documentation and follow-up.
