Job summary

Foundation Medical Partners in Nashua, NH seeks an APRN or PA to join our Neurology team in a full-time outpatient role. You will evaluate and manage patients with a broad range of neurological conditions, with 36 clinical hours and 4 administrative hours per week, and collaborate with a multidisciplinary team.

The role requires current NH APRN or PA licensure, strong clinical and communication skills, and a commitment to patient-centered care.

Qualifications

• APRN or PA license in New Hampshire (active or eligible)
• Strong clinical, diagnostic, and communication skills
• Experience in outpatient neurology is preferred
• Proven ability to work in a team-based care setting

Responsibilities

• Provide outpatient evaluation and management of patients with general neurological disorders
• Work 36 clinical hours with 4 hours of administrative time weekly
• Diagnose and treat a wide range of neurological conditions (e.g., neuropathy, headaches, epilepsy)
• Collaborate with a multidisciplinary care team to ensure high-quality patient care
• Document accurately in EPIC and participate in call rotation as required
• Engage in quality improvement initiatives and CME opportunities
