• Manage all case provisions for active rehabilitation caseload, covering residential neurorehabilitation and day treatment.
• Lead team meetings and coordinate treatment goals with the treatment team; assist with discharge planning.
• Ensure implementation of treatment plans from admission to discharge and follow-up.
