HonorHealth is seeking a Neuro-Oncology Nurse Navigator (RN) to coordinate care for neuro-oncology patients and families across the continuum. You will intake neuro-oncology referrals from local, national, and international sources and provide consultation, support, and education to patients diagnosed with neuro-oncology tumors.

The role requires strong independent motivation, clinical expertise, and a commitment to quality care.

Formación

• Bachelor's in Nursing required.
• Master's in Nursing or Master's in Healthcare Administration/ Business Administration preferred.
• 3 years in oncology or neurology required.
• 5 years in Nursing required.
• 5 years in oncology or Neurology with demonstrated expertise in neuro oncology care, and knowledge of post-operative patient process preferred.

Responsabilidades

• Assesses patient and family complex needs. Identifies barriers to care and supports resolution in person and remotely.
• Facilitates referrals to interdisciplinary team members and scheduling diagnostic studies and follow-ups.
• Assists with transition into survivorship including group meetings and education.
• Provides extensive patient/family education and support via face-to-face, electronic media and phone consultations.
• Organizes continued care after consultation based on plan of care; evaluates symptoms related to cancer or treatment and suggests symptom-management.
• Facilitates relationships with neuro-oncology colleagues and prepares tumor boards with records maintained per best practices.
• Acts as liaison to patients, health care providers and organizations to promote seamless care.
