Qualifications

• Assistance with activities of daily living (ADLs) and basic patient care.
• Ability to measure and document vital signs.
• Effective written and verbal communication with patients and care team.

Responsibilities

• Excellence in Patient Experience by partnering with health care team, patient and family.
• Patient Centered Care—Clinical Practice and collaboration with the plan of care.
• Provide patient care under the direction of the RN.
• Assist RN and healthcare team with care delivery as assigned.
• Monitor data entry for key patient safety information in EMR.
• Participate in charge reconciliation and audits as requested.
• Transport unit supplies, medications, and equipment as assigned.
• Adhere to safety and infection control practices and HIPAA regulations.
