Job summary

Live Well Homecare is seeking a Physician Assistant or Nurse Practitioner to join a hospital-based nephrology team, providing inpatient and outpatient care on a hospital campus.

You will conduct inpatient rounds, assist with dialysis, and support consults and follow-ups for AKI and electrolyte issues, while collaborating with physicians to deliver comprehensive kidney care on a busy service.

Qualifications

• Bachelor's Degree in Physician Assistant studies required; Master's preferred.
• Training in Nephrology is not necessary, as on-the-job education will be provided.
• The candidate should enjoy working in a fast-paced, team-based environment.

• Conduct inpatient rounding, assist with outpatient dialysis, support new consults and follow-ups for acute kidney injury and electrolyte issues, evaluate dialysis patients, assist with inpatient charting, and support outpatient clinics.
• Manage inpatient and outpatient nephrology cases, participate in on-call rotation (one weekend/month), and collaborate with physician partners to deliver comprehensive kidney care.
