• Dexterity to operate complex medical equipment and document care.

Responsibilities

• Apply Case Management Standards of Practice to manage care for assigned patients.
• Assess needs, plan treatment, and coordinate with family and care team.
• Facilitate communication among family and team; advocate to overcome barriers.
• Develop and implement care plans to meet goals and optimize resources.
• Document interactions and participate in team conferences.
• Lead program development and quality improvement initiatives.
