Qualifications

• Current RN license in the state of practice.
• BLS certification required.
• Five years of direct patient care experience.
• BSN degree within three years of hire if applicable.

Responsibilities

• Apply Case Management Standards: Assess, Plan, Facilitate and Advocate for assigned patients.
• Assess patient/family needs, goals, and barriers to care with the care team.
• Plan with patient/family and team to optimize outcomes and care delivery.
• Facilitate communication between family and care team and document interactions.
• Advocate to resolve barriers using available resources and processes.
• Collaborate with physicians, patients, and team members to set goals and interventions.
• Identify acuity levels and coordinate care across settings.
