RenalCare Associates is seeking an experienced Registered Nurse to join as a Nephrology Nurse Navigator. The role centers on patient education, care coordination, and guiding individuals with chronic kidney disease through care decisions.

Formación

• Active RN license in good standing.
• ASN or BSN required.
• Experience in nephrology, dialysis, transplant care, chronic disease management, or related field preferred.
• Strong clinical judgment and organizational skills.
• Excellent communication and patient-education focus.
• Ability to work independently while collaborating with physicians.
• Experience with electronic health records.

Responsabilidades

• Educate patients and families about chronic kidney disease, disease progression, and treatment options.
• Explain dialysis modalities (in-center, home, peritoneal) and transplantation.
• Coordinate referrals and follow-up for transplant evaluations and dialysis access planning.
• Collaborate with nephrologists, clinic staff, dialysis centers, and other providers.
• Monitor and track patient care milestones, labs, referrals, and treatment plans.
• Identify barriers to timely care and connect patients with resources.
• Serve as a consistent point of contact for patients and families.
• Maintain accurate documentation and communicate updates to the care team.
