Job summary

RenalCare Associates SC is seeking an experienced Registered Nurse to join as a Nephrology Nurse Navigator. The role centers on patient education, care coordination, and guiding individuals with chronic kidney disease through care options and next steps.

You will collaborate with nephrologists, clinical teams, and partners to support progression through kidney disease with confidence and continuity of care. Full-time, on-site in Peoria, IL.

Qualifications

• Active RN license in good standing with state authorization.
• Experience in nephrology, dialysis, or transplant care preferred.
• Strong communication and patient education abilities.

Responsibilities

• Educate patients and families about kidney disease and treatment options.
• Explain dialysis modalities and transplant evaluation processes.
• Coordinate referrals and follow-ups with nephrologists and clinics.
• Track patient milestones, labs, and care planning per protocols.
• Identify barriers to timely care and connect to resources.
• Serve as a consistent contact for patients navigating decisions.
