Job summary

Hugh Chatham Memorial Hospital in Oswego, IL is seeking a Nephrology Advanced Practice Provider (NP/PA) to join a four-nephrologist team and care across inpatient, SNF, and outpatient settings in the service area.

The APP conducts patient assessments, develops treatment plans with physicians, participates in rounds at Rush Copley and local sites, and provides outpatient follow-up in Oswego two half-days weekly.

Qualifications

• Active RN, and APRN license in state(s) of practice.
• Active national board certification in area of clinical practice and populations served.
• Active DEA registration prior to hire.
• If Illinois practice: active Illinois Controlled Substance License prior to hire.
• Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire.

Responsibilities

• Assess, diagnose, and manage care including ordering and interpreting diagnostic studies and medications.
• Collaborate with nephrologists to develop and monitor individualized treatment plans.
• Record encounters and treatment plans in the EMR accurately and efficiently.
• Coordinate care with physicians, nursing staff, specialists, patients, and families.
