Responsibilities

• Assesses the physical and psychosocial status via interview, history, exam and diagnostics.
• Formulates and implements treatment plans in collaboration with the medical team.
• Documents inpatient H&P and discharge summaries; countersigned within 24 hours.
• Participates in daily multidisciplinary rounds with care teams.
• Ensures care complies with practice protocols and Baptist Health policies.
• Collaborates with other health professionals to ensure continuity of care.
