Qualifications

Responsibilities

• Assess patient's physical and psychosocial condition on an ongoing basis.
• Develop, communicate and document an individualized plan of care with family and healthcare team.
• Implement and coordinate the plan of care with the healthcare team.
• Evaluate plan effectiveness, patient response, and discharge readiness; revise as needed.
