Job summary

Providence Portland Medical Center's Franz Clinic is seeking an Oncology Nurse Navigator (ONN) to support cancer patients across the continuum of care. This RN will coordinate care, triage needs, and offer information and education to patients and families during the cancer journey.

Primary goals include empowering patients to participate in their care, reducing stress through clear guidance, and guiding survivorship planning when applicable, within Providence's patient-focused culture of

Qualifications

• RN license in Oregon required at hire.
• National Provider BLS - American Heart Association at hire.
• Oncology experience: minimum 3 years.
• OCN must be completed within 18 months of hire.

Responsibilities

• Serve as a consistent resource throughout the continuum of cancer care.
• Triage physical, psychological and social needs of patient and family.
• Provide information and education to reduce stress and empower patient.
• Assist with survivorship planning when applicable.

The Oncology Nurse Navigator (ONN) serves as a consistent resource throughout the continuum of cancer care. This RN is knowledgeable on the multiple treatment decisions, strong emotions and financial impact of a cancer diagnosis. The ONN will triage the physical, psychological and social needs of the patient and significant others, offering intervention and /or referrals as needed. Primary goals of this position are to: 1) enable patients with information and education thus reducing a portion of the stress involved in a cancer diagnosis, 2) empower the patient to participate in their personal care process, 3) improve understanding of their decisions and treatment plans; and 4) complete the treatment though the point of survivorship planning (when applicable).
