The Interventional Cardiology Nurse Navigator serves as a consistent resource to support optimal care delivery for cardiology with a focus on interventional cardiology and STEMI patients as well as other projects as assigned This nurse navigator takes on a leadership role in developing the coordination of care during pre-procedure, hospitalization, discharge planning and facilitating post-hospitalization follow-up.
Works in collaboration with the providers and clinics to assure a smooth transition of patients during preadmission, hospitalization, preparation for discharge and outpatient services to achieve the best possible care plan.
- Assists with monitoring the identified care delivery best practices and collaborates with the clinical team to identify and implement care redesign to improve clinical outcomes.
- Navigates and coordinates major aspects of care delivery, using appropriate skills for patients in need of information, education and resources.
- Emphasizes empowerment of patients through education and support. Promotes patient advocacy during the hospitalization and post discharge.
- Responsible for initiating oral and written communications to the appropriate team members in a timely manner. Responsible for communicating observations to the appropriate members of the interdisciplinary health care team.
- Participates in key quality activities, as appropriate.
- Exhibits skill in prioritizing tasks and assists co-workers with duties as needed in a positive manor.
- Co-facilitates development of patient teaching materials, as needed.
- Collaborates with the clinical team to assure that the care of the indigent client is not lost to follow up and gains access to necessary medical care.
- Participates in data collection and reporting of outcomes as requested.
- Collaborates in development/updating of policies and procedures specific to Nurse Navigation.
- Builds relationships with the practitioners to enhance communication, to achieve improving the patient and practitioner experience.
- Provides nursing inpatient and outpatient staff updates on key issues.
- Assists with other projects within the Providence Heart and Vascular Institute (PHVI) as directed.

Required Qualifications
- Bachelor's Degree - Bachelor's degree in nursing (BSN) from an accredited school.
- Washington Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 3 years - Experience as a registered nurse in cardiovascular services.
