In this essential role, you will be the cornerstone of comprehensive care coordination, serving as the primary point of contact and ensuring seamless navigation through our health system. You’ll work collaboratively with providers and clinical teams to deliver patient-centered solutions.

What You’ll Do

• Care Coordination: Facilitate interactions between patients and their clinical care team, ensuring a smooth and coordinated experience throughout the continuum of care.
• Patient Education: Empower patients, families, and caregivers by educating them about diseases, treatments, side effects, and how to access reliable information.
• Resource & Support: Collaborate with providers to implement care standards, and identify/secure appropriate services. You’ll connect patients with crucial resources for psychosocial distress or tangible barriers to care (e.g., finances, housing, transportation), referring to social workers or counselors as needed.
• Clinical Access: Provide resources for specialized treatments or testing not available locally, and promote awareness of appropriate clinical trials.
• Patient Empowerment: Encourage patients, families, and caregivers to take an active role in managing their care, within the scope of their individual circumstances.

Pay Range
