Job summary

Community Bridges seeks a Navigator II in Tucson, AZ, to enhance client success by identifying barriers and connecting individuals with needed services. You will engage clients, provide case management, and document interactions within state and agency guidelines.

You will work on a multidisciplinary team, map next steps, and support clients toward sustained recovery and stable access to resources. Growth opportunities and a strong benefits package accompany the role.

Qualifications

• Must have completed at least 12 months recovery from substance use and/or mental health disorders.
• 21 years of age or older.
• Arizona Fingerprint Clearance card (program specific).
• Current AZ Driver’s License (valid and in good standing).
• Ability to provide a 39-month motor vehicle record (MVR).

Responsibilities

• Conduct outreach, engagement, and ongoing wrap-around supportive services.
• Coordinate case management and help clients access needed services and resources.
• Document and track required patient interactions per AAC, ADHS/DBHS, CARF, and internal policies.
• Map out the next steps for clients to achieve ongoing success as part of a multi-disciplinary team.

Job Summary

The Navigator II’s primary role is to enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources. The Navigator II will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services. The Navigator II will also map out the most appropriate next steps for the individual who may need assistance in accessing various services to achieve ongoing success. The Navigator II is part of a multi-disciplinary team made up of internal and external stakeholders. The Navigator II will document, and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific for the program.

Skills/Requirements

• Highschool diploma or GED is required. Associate degree (or higher) in a field related to Behavioral Health is preferred but not required of this position.
• Minimum of twelve months of recovery from substance use and/or mental health disorders required.
• 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
• 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
• Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required.
• Must be at least 21 years of age.
• Arizona Fingerprint Clearance card (Program Specific).
• Current AZ Driver's License (valid and in good standing).
• Must be able to provide a 39-month MVR.
