Job summary

Responsibilities

• Community Liaison and Outreach: identify Native families, conduct home visits, serve as liaison between patients and health care team.
• Care Coordination: assist eligible community members in obtaining services, follow up on referrals, visit hospitals or care facilities, coordinate health education.
• Transportation Support: arrange patient transport and manage transport vehicles.
• Documentation and Reporting: maintain daily logs, confidential records, and participate in training and meetings.
• Professional Development and Collaboration: attend workshops and coordinate with clinic staff.
