Job summary

Altra Homecare Group is seeking compassionate individuals to join our team as PCA, HHA, CNA, or those with no prior experience for in-home personal care. You will oversee delivering essential care services tailored to each patient’s needs and ensure safety during daily activities.

Responsibilities include medication reminders, light meals, mobility assistance, ROM exercises, documentation, and companionship.

Qualifications

• Must be 18 years of age or older.
• Possess a valid Driver's License and Auto Insurance.
• Ability to read, follow written instructions, and document care provided.
• Comply with training requirements as per State and Federal laws.

Responsibilities

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety.
• Report significant changes in the patient's condition directly to their supervisor.
• Prepare light meals and assist with patient feeding when required.
• Monitor and record patient temperature.
• Assist patients with ambulation.
• Conduct range of motion exercises and other simple physical activities.
• Maintain comprehensive documentation and adhere to care protocols.
• Follow privacy policies and infection control guidelines.
• Offer bathing assistance.
• Perform light housekeeping.
• Provide companion services, including letter-writing and reading.
