Job summary

LiveWell Homecare Agency in Atlanta, GA seeks an experienced Registered Nurse to provide high-quality, personalized nursing care in a multi-campus homecare setting. The RN will own the nursing process from assessment to evaluation, building meaningful connections with patients and families and ensuring evidence-based outcomes.

Qualifications

• Graduate of an accredited nursing program (required)
• BSN preferred
• RN license valid in appropriate state(s) and BLS/ACLS certifications
• 2+ years Med Surg experience with proficiency in data entry
• Familiarity with Epic and 3M systems is preferred

Responsibilities

• Deliver patient-centered nursing care following evidence-based practices
• Foster a safe, collaborative team environment and support performance improvement
• Guide and mentor colleagues, supporting professional development
• Lead application of evidence-based practices and participate in safety initiatives
• Plan resources and develop protocols to improve patient experience and reduce readmissions
