Job summary

AIDS Healthcare Foundation (AHF) is seeking a Clinical Pharmacist – Multisite to expand primary care functions across multiple sites, providing immunizations, long-acting HIV treatments, diabetes monitoring, nicotine reduction programs, and other SME services in collaboration with AHF providers.

This role supports best-practice implementation and ensures consistent processes and policies throughout the organization, contributing to AHF's mission and patient care across locations.

Qualifications

• Licensed pharmacist with state license.
• Experience administering vaccines and immunizations.
• Experience working across multiple sites is a plus.

Responsibilities

• Provide immunizations and clinical services across multiple sites.
• Support diabetes monitoring and smoking cessation programs.

A Clinical Pharmacist – Multisite contributes to AHF by expanding primary care functions of pharmacists at more than one site to provide individualized health promotion and disease prevention, including administration of vaccines, long-acting HIV treatments, diabetes monitoring, nicotine reduction programs and other subject matters, in collaboration with other AHF providers.

A Clinical Pharmacist – Multisite enhances the reputation of AHF by providing valuable contributions to more than one location by bringing “best practice” and collaborative ideas, as well as providing consistent application of processes, procedures and policy throughout the organization.

The Clinical Pharmacist – Multisite will work as a part of a close-knit team to provide individualized health promotion and disease prevention, including administration of immunizations, diabetes and smoking cessation programs, as well as other Subject Matter Expertise (SME) programs as developed by AHF, with appropriate collaborative drug therapy management authority. This includes prescriptive authority where this is legally and organizationally authorized such as PrEP and PEP therapies
