Job Overview

Job Location: Yuba City, CA 95991. Position Type: Part Time. Job Shift: Day. Salary Range: $90.00 per hour. Job Category: Health Care – Medical Social Worker, Home Health (part‑time).

Compensation includes competitive pay, mileage reimbursement, 3 weeks of PTO per year, 401(k) match after one year, and referral bonus up to $500.

Responsibilities

• Provides psychosocial evaluation, diagnosis, treatment, and referral for patients and/or families. Evaluation shall include patient and/or family goals.
• Interprets results of evaluation and recommends treatment as indicated to the patient/family referral source.
• Provides services that include but are not restricted to community referral, individual therapy, family therapy, group therapy, consultation, or education.
• Aids the health care team in identifying, understanding and integrating psychosocial factors in relation to the patient’s illness, treatment, and recovery.
• Plans and leads individual and family therapy, patient and family group sessions, and family conferences.
• Provides a written report for patient medical record (evaluation, progress notes, and discharge summary) with copies to referral source, patient, and family as indicated.
• Maintains accurate, up‑to‑date records in the client chart.
• Consults with physicians, families, and referral sources regarding client progress and therapy program.
• Maintains all departmental records in accordance with company policy, state and federal regulations.
• Maintains confidentiality of client’s situation, records and services provided in accordance with HIPAA regulations.
• Coordinates total patient care and discusses with the Director of Clinical Services solutions for patient needs and team involvement.
