Job summary

Rochester Regional Health is seeking a Social Worker for the Palliative Care Services at Homecare Lakeview in Lakeville, NY. This full-time role delivers psychosocial support, counseling, and care planning to home care clients, coordinating with the health care team and documenting activities.

Responsibilities include psychosocial assessments, discharge planning, coordinating community resources, and liaison with social work and community agencies.

Qualifications

• A Master’s degree in Social Work with a minimum of two years’ experience in Social Work or related field (level I).
• Proven psychosocial assessment skills.
• Effective verbal and written communication skills essential.
• Ability to stay organized and flexible; possession of a valid NYS driver’s license and own car.
• Pre-employment physical exam as required by NYS Dept. of Health.
• Demonstrates strong communication, interpersonal, organizational, and time management skills.
• With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.
• Demonstrated ability to effectively manage time and caseload autonomously.
• On Call may be required.

Responsibilities

• Conduct psychosocial/mental health assessments related to illness of patients.
• Interpret social factors to the home health team that may affect progress.
• Assist patients and families in understanding and planning for illness adjustments.
• Coordinate resources to alleviate financial impact of illness and payment planning.
• Determine need for continued social work service and connect with community agencies.
• May assist with orientation of new staff and discharge planning from Lifetime Care programs.
