Job summary

Adoration Health in Altoona, PA is seeking a compassionate Social Worker to support patients facing terminal illness with emotional care and resource coordination.

You will assess needs, provide counseling, educate on advance directives, and liaise with community resources to ensure dignified, holistic care for patients and families at home or in hospice settings.

Qualifications

• Master's in social work (MSW) or Bachelors in social work (BSW) CSWE accredited.
• Minimum 1 year healthcare/hospice experience preferred.
• Current licensure to practice in state of operation; licensed driver with insured vehicle.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety and assist with alternative care plans.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team to support holistic patient care.
• Support funeral, financial, and legal planning with families.

Our Social Worker s provide emotional support, counseling, and resource coordination to patients facing terminal illness. Qualifications

• A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in
• 418.114(b)(3).
• Minimum of one (1) years' experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
