Qualifications

• Bachelor's degree in a social work program accredited by the CSWE.
• MSW licensure by the SC Board of Social Work Examiners.
• Certification in case management preferred.

Responsibilities

• Interviews patients and relatives to obtain social history relevant to medical problems and planning.
• Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care.
• Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies.
• Interprets and assists in resolving social problems that relate to medical condition and/or hospitalization.
• Assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team.
• Completes an assessment on the patient and caregivers during hospitalization and follow-up as needed.
• Identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower risk of undesirable events post discharge.
• Bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
