Job summary

Compassus in Washington state is seeking an experienced Social Worker to join our home health team, coordinating care for patients and families in community settings. The role emphasizes autonomy within guidelines and collaboration with healthcare professionals.

You will assess psychosocial needs, deliver crisis intervention, ensure regulatory-compliant documentation, and support discharge planning while working with an interdisciplinary team to maximize patient functioning in the community.

Qualifications

• Master's Degree from an accredited School of Social Work.
• WA/TX: 1 year post-Master’s experience in a health care setting (preferred).
• AK: 2 years post-Master’s experience in a health care setting (preferred).
• BLS (American Heart Association) certification (Vendor Managed).
• Washington Advanced Social Worker License or Clinical Independent Social Worker License (or Associate).

Responsibilities

• Assess patients’ psychosocial, financial, environmental, and community needs.
• Provide direct and indirect social services support for patients and families in homes, long-term care facilities, hospitals.
• Deliver crisis intervention and supportive counseling.
• Complete documentation in compliance with regulations and policies.
• Participate in interdisciplinary team and care planning.
• Coordinate patient/family services and discharge planning.
• Respond to referrals from the team and community.
• Promote teamwork and professional conduct.

Education

Master's Degree in Social Work

Washington Advanced Social Worker License or Clinical Independent Social Worker License (or Associate)

BLS Certification (American Heart Association)
