Job summary

The University of Texas Health Science Center at San Antonio is seeking a licensed social worker to provide advanced social services, case management, and program development within UT Health San Antonio system. You will perform psychosocial assessments, coordinate care, and support patients from diverse backgrounds.

Requirements include a Master's in Social Work, LMSW/LSW license, and at least three years of professional social work experience.

Qualifications

• Excellent communication skills, both verbally and written; bilingual (Spanish/English) preferred.
• Ability to independently conduct psychosocial assessments and provide psychosocial treatment.
• Knowledge of human development, environment, culture, and health systems.
• Experience with medical/mental health diagnoses and treatments.
• Independent judgment and crisis intervention skills.
• Experience using Microsoft Office and data management software.

Responsibilities

• Performs psychosocial assessments, health education, supportive counseling and referrals; assists patients in finding resources.
• Collaborates with multidisciplinary team to provide comprehensive care; documents all patient/provider communication and interventions.
• Identifies obstacles to optimal patient care; risk reduction; and health promotion.
• Provides clinical case management services including referrals for housing, meal assistance, medical transportation, and other needs.
• Formulates candid and supportive communication.
• Coordinates ongoing counseling and support groups.
• Facilitates effective problem solving and conflict resolution.
• Participates in program evaluation activities including data collection for grants and patient satisfaction surveys.
