Qualifications

• Master of Social Work (MSW) is required.
• Experience in adolescent social assessment and intervention is preferred.
• Ability to document in medical records and coordinate with services.

Responsibilities

• Assess adolescents' social and emotional needs and develop shared plans.
• Provide concrete services to clients and families or connect them with needed resources.
• Record relevant data in the patient’s medical record.
• Conduct short-term, goal-oriented social work interventions for multiple cases.
• Identify and refer to external resources and monitor contracted services.
• Offer HIV counseling and ongoing case management as needed.
• Support prenatal, family planning and primary care programs with psychosocial assessment.
• Provide health education through individual and group sessions.
• Participate in staff development and CHN committees as assigned.
• Coordinate confidentiality and privacy in all activities.
