Job summary

POSITION SUMMARY - MSW Social Worker

POSITION DUTIES - Assesses patients' psychosocial needs, provides counseling and crisis intervention, coordinates discharge planning, and connects patients and families with community resources to support safe and effective care transitions.

Minimum Required Qualifications -

• MSW
• 2 years recent social work experience in an inpatient acute hospital setting

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

Monday through Friday 8:00A to 4:30P, no call. Weekend rotation of one weekend day per month.
