Job summary

21st Century Home Health Services (21HHS) is seeking a Medical Social Worker for in-home visits in the Auburn area. You will assess social and emotional factors, educate families, and coordinate with an interdisciplinary team to ensure seamless patient care.

Primary location is Auburn with occasional coverage in surrounding counties. Applicants must hold a CSWE-accredited MSW; BLS certification is required.

Qualifications

• Knowledge of local community resources is required.
• Demonstrated communication skills (written and verbal).
• Core competencies and time management essential.
• LCSW license preferred; supervision provided to license-eligible social workers.
• Must maintain licensure and meet professional development needs.

Responsibilities

• Assess social and emotional factors related to the patient's illness in the home and develop a plan for home social work services.
• Coordinate with case managers, families, and other home health staff to integrate care.
