Job summary

21st Century Home Health Services is seeking a Medical Social Worker (MSW) to support geriatric patients in home care. You will assess social and emotional needs, educate families, and coordinate with the interdisciplinary team to implement care plans in patients' homes.

The role requires an MSW degree, active BLS certification, and state licensure eligibility. We value flexibility, strong communication, and the ability to navigate local resources; occasional travel within the assigned territory

Qualifications

• MSW degree from a program accredited by the Council on Social Work Education (CSWE).
• BLS certification required.
• Driver license required.
• LCSW preferred (supervision provided to license-eligible).

Responsibilities

• Assess social and emotional factors related to the patient’s illness and develop a home social work plan.
• Coordinate with the Case Manager and interdisciplinary team to integrate care and educate families.
