Job summary

Virginia Mason Franciscan Health is seeking a Hospice Care Social Worker to provide comprehensive psychosocial support and counseling to hospice patients and their families in the Puget Sound region.

Responsibilities include assessments, care planning, grief support, and resource navigation, with advocacy for patient needs and collaboration with the hospice team to deliver end-of-life care with compassion.

Qualifications

• Masters degree in Social Work required upon hire.
• WA licensure as LICSW or equivalent required upon hire.
• Additional WA licenses considered an asset.

Responsibilities

• Provide psychosocial support, counseling, and resource navigation to hospice patients and families.
• Assess, develop care plans, offer emotional and grief support, and connect families with resources.
• Advocate for patient needs and collaborate with the hospice team; conduct family conferences as needed.
• Plan interventions, facilitate safe discharge planning, and evaluate outcomes of care.
