Angels Care Home Health is seeking a Medical Social Worker (MSW) to provide psychosocial assessments and coordinated home health care across age groups in Sun City West, AZ. The role emphasizes counseling, resource referrals, and family support within a multidisciplinary team.

The MSW will assess patient needs, maintain up-to-date documentation, and participate in care planning while meeting continuing education requirements and upholding agency policies.

Responsibilities

• Assess home health patient to identify psychosocial, financial, environmental, and community needs as evidenced by POC, documentation, clinical records, case conferences, and community resource referrals.
• Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
• Meet mandatory continuing education requirements
• Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards.
• Promote Agency philosophy and administrative policies to ensure quality of care.
